It is explicitly stated in the story that the charater of Akane is based on Pinoko from Osamu Tezuka's Black Jack - Kohei's activities as an unlicensed surgeon are also based on Black Jack.

The Leviathan, which is Hebrew for "sea monster", is a mythical sea serpent that appeared in the Bible. It is also the name of a book about government and morality written by Thomas Hobbes in the 1600s. The book offers a rather pessimistic view of humans and politics, saying that people must be ruled by an absolute monarch because they are driven by fear and violence and that the very nature of man is evil.
