Daisuke INOUE (井上 大輔 ) is 3 persons with only 2 kanji names:

----------------------------------------------------------------------------------------------------
井上 大助 = いのうえ だいすけ = INOUE Daisuke (manga-ka) (born 1952, Wikipedia page, splitting him from a 1935 born actor with the same kanji name)
井上 大助 (1991) Mobile Suit Gundam F91 (manga) : Story & Art (http://shop.kodansha.jp/bc2_bc/search_view.jsp?b=3531333)
(split) The contributor of this credit even went so far to give the kanji name in the precision field. ("Daisuke" is one of 4 possible readings.)
----------------------------------------------------------------------------------------------------
井上 大輔 = いのうえ だいすけ = INOUE Daisuke (musician/composer) (1941-09-13 - 2000-05-30, Wikipedia page

井上 大輔 (1985) Musashi no Ken (TV) : Theme Song Composition (Wikipedia)
井上 大輔 (1987) Kimagure Orange Road (TV) : Theme Song Composition (http://anison.info/data/program/373.html)
井上 大輔 (1990) Sword for Truth (movie) : Theme Song Composition ("Credits"; better: http://anison.info/data/song/4149.html)
井上 大輔 (1993) Kiss wa Me ni shite (OAV) : Theme Song Composition (inutori bekkan)
井上 大輔 (1998) Silent Möbius (TV) : Theme Song Composition ("Anime Unleashed (TechTV)"; better: http://anison.info/data/song/15349.html)
(keep) This is the current owner of this entry. ("A Forbidden Pensee" was finally his masterpiece...)
----------------------------------------------------------------------------------------------------
井上 大輔 = = INOUE Daisuke (photographer)
(same kanji as the composer; 2 ENAMDICT readings for 大輔 = Daisuke, Taisuke; ANN has ca. 50 大輔, all "Daisuke")
井上 大輔 (1999) Samurai X: Trust & Betrayal (OAV) : Photography (D2_STATION)
_________ (1999) Legend of Himiko (TV) : Photography (no source)
_________ (2002) Beat Angel Escalayer (OAV) : Photography ("DVD credits")
_________ (2003) Slave Nurses (OAV) : Photography ("Credits")
井上 大輔 (2004) Monster (TV) : Digital Photography (T Nishimura) (D2_STATION)
_________ (2006) Shinigami no Ballad: momo the girl god of death (TV) : Photography (http://www.momo100100.com/staff/index.html, high-level only)
井上 大輔 (2006) Animal Crossing (movie) : Photography (T Nishimura) (livedoor wiki)
井上 大輔 (2007) Eiga de Tōjō! Tamagotchi Doki Doki! Uchū no Maigotchi!? (movie) : Photography (Tripod) (livedoor wiki)
井上 大輔 (2008) Eiga! Tamagotchi Uchū Ichi Happy na Monogatari!? (movie) : Photography (Tripod) (livedoor wiki)
井上 大輔 (2009) Pokémon: Arceus and the Jewel of Life (movie) : Photography (Tripod) (livedoor wiki)
井上 大輔 (2010) Pocket Monster Diamond & Pearl: Genei no Hasha Zoroark (movie) : Photography (Tripod) (livedoor wiki)
(split) Same kanji as the composer who died in 2000 already, so this can't possibly be the same person.

One more additional credit for the camera operator:
井上 大輔 F-Zero Falcon Densetsu (TV) : Digital Photography ("file:///home/tae/Documents/animedatabase/F-ZERO.html"; better: shirayuki)

Instead of posting every single additional credit to this thread, just add the kanji name (as prefix) to the source URL.
When Dan42 will perform the split he will see this source string and then know which person the credit belongs to.

Sometimes I even add kanji names to existing credits (in the form of an endorsement) in order to note the information for an obvious future split request that I don't have the time to make at the moment (when the given source is useless but I happen to have a better source at hand).
