Yuko OTA appears to be two people:

太田裕子:
staff (Finishing) for End of Evangelion, 1997 (kanji source)
staff (Finishing) for Gasaraki (TV), 1998 (no source)
staff (Paint) for Digimon: The Movie, 2000 (source: "Credits")

大田優子:
cast for The Door Into Summer (movie), 1981 (kanji source)

Um... I just found this page for the seiyuu:
http://movie.goo.ne.jp/cast/86679/index.html
where her name is given as
大田優子 (オオタユウコ = OOTA Yuuko).

Would it be possible resp. helpful to adapt the romanization of her name?
(Sorry that I didn't have this idea earlier, I just found this page in the process of contributing information to the seiyuu entry that didn't exist before so that searching for this stuff wouldn't have been contributable).

While we standardised fairly recently on revised hepburn, the encyclopedia uses all manner of different schemes (often for the same person) depending on who entered the information and what source they were using (and in some cases, what the person themself uses)

They are likely both おおた - おた isn't even a valid reading for 太田.
