Most credits are based on the English DVD cover as the only source so far. Based on the Japanese Wikipedia page for this serie, a Kanji source for all cast, another Kanji source with Furigana for all Seiyuu names and a Kanji source for a lot of staff they reveal a couple of Kanji misreadings, thus causing a number of duplicate persons whose only staff entry is this anime:

Mina TOMINAGA (no kanji, 1 cast) is a different romanization duplicate of
Miina TOMINAGA (冨永みーな, >100 cast)
みーな expresses a long "i", i. e. "Miina" is better than "Mina".

Koichi YOKOTA (no kanji, 1 cast) is a different romanization duplicate of
Kouichi YOKOTA (横田紘一, 3 cast).
紘一 can be "Kouichi"; "Koichi" would require different Kanji.

Makoto ITO (井藤誠, 28 staff, Key Animation, plus 1 cast) is a different romanization duplicate of
Makoto ITOU (井藤誠, 2 staff, Key Animation)
which is the better romanization for いとう as ITO would require different Kanji.

Kumiko KATO (no Kanji, 3 staff, Animation tasks) might be a different romanization duplicate of
Kumiko KATOU (加藤久美子, 6 staff, Animation tasks).
Unfortunately, both KATO and KATOU are valid names with different Kanji. 加藤 is KATOU with a long "o", so this one looks correct. KATO Kumiko has no Kanji sources given. But she is credited for "Animation" in MADLAX. Let's google for a decent "マドラックス" source... this one looks fine. And in fact, there's a credit for 原画 to the person 加藤久美子... so the KATO person is actually a duplicate of KATOU (based on the MADLAX credit, I didn't check the others as well).

Masaaki KUDO (工藤柾輝, 10 staff, Key Animation) is a Kanji reading error duplicate of
Masaki KUDO (工藤柾輝, 9 staff, Animation director).
"Masaki" is the only valid reading for 柾輝 according to EDICT.

A [url="http://www.google.de/search?q=%22柳瀬雄之%22+%22やなぎせ%22"]Google search for "柳瀬雄之" "やなぎせ"[/url] produces 0 hits, so YANAGISE might be less likely than YANASE. A search for "雄之" in ANN produces two hits, with Takeyuki and Yuji as given names (EDICT allows Takeyuki, Hiroyuki and Yuushi).

Masaru KINJOU (金城優, no credits at all) is apparently a Kanji reading error duplicate of
Yuu KANESHIRO (金城優, 4 staff for Animation).
Both names would be valid readings for these Kanji. But as Masaru KINJOU apparently has already been stripped of his credits he should lose his Kanji name as well, in order not to confuse people who enter Kanji data and hope for a unique match of these.

Takeshi SHUKURI (宿利剛, 5 staff for Music Production) appears to be a different romanization duplicate of
Gou SHUKURI (宿利剛, 2 staff for Music Production)
as 剛 allows both readings and the Japanese video gives credit to 宿利剛 while the U.S. video gives credit to Gou SHUKURI (which may or may not be correct but at least they think they're the same person). The Allcinema page for 宿利剛 has five credits (including "Beck" and "Da Capo S.S.", both credited to Takeshi SHUKURI at ANN) but gives a reading of しゅくり (SHUKURI) ごう(Gou), thus confirming the information from the U.S. DVD of Suzuka.
And some other persons got credited for tasks by people with different Kanji names:

Maki SAITOU (齋藤真紀, 2 staff as Color Setting/Digital Paint, 7 cast) appears to be more than one person.
The staff person in Suzuka (TV) has different Kanji 斉藤麻記 whose logical reading would also be SAITOU (4 choices) Maki (only possibility).
I refrained from creating a new person SAITō Maki whom I could have credited for Digitial Paint in Suzuka as this wouldn't have dealt with the already existing staff credit to SAITOU Maki and rather caused additional confusion instead.

The staff person in Suzuka (TV) has different Kanji 斉藤友子 whose logical reading would also be SAITOU (4 choices) Yuuko/Tomoko/Mitsuko.
There's already a SAITOU Tomoko in the ANN database who again has cast roles and staff functions (Finish and Recoding) but her Kanji name is 斎藤朋子 as the Kageman TV source for her Recording credit points out. The only Finish staff credit unfortunately has no Kanji source and might thus belong to 斉藤友子 instead.

Shigeru CHIBA (千葉繁, 4 staff for Animation tasks, ca. 20 staff for Sound/Recording, >100 cast roles) appears to be more than one person.
The Animation person has a kanji name of 千葉茂 (as shown in the D2_STATION stats for Suzuka TV) whose reading might also be Shigeru CHIBA (amongst many other possibilities in EDICT but statistically these are the plausible readings based on the ANN database content).

Takayuki YAMAGUCHI (山口隆行, 7 staff for Sound/Recording, 4 staff for Original Story/Art, >25 cast roles) appears to be three different persons.
The Sound/Recording person has a kanji name of 山口貴之 (as shown in the D2_STATION stats for Suzuka TV) whose reading would be YAMAGUCHI Takayuki/Takashi. (The Wikipedia page for these Kanji is a soccer player so it's difficult to find sources for the reading of this Sound/Recording person's name.)
The Mangaka has his own Wikipedia page, a birth date of 1966-11-01 and a Kanji name of 山口貴由 with a reading of やまぐち (YAMAGUCHI) たかゆき(Takayuki).
The Seiyuu has his own Wikipedia page, a birth date of ????-12-28 and a Kanji name of 山口隆行 with a reading of やまぐち (YAMAGUCHI) たかゆき(Takayuki).
- There might be a tricky problem with the pair of
Kouji AOKI (青木孝司, 12 staff for Photography) and
Takashi AOKI (青木隆, 11 staff for Photography).
Yes, they do have different Kanji names (note that 孝司 allows the readings Takashi, Takaji, Koushi and Kouji). But the Japanese video for Suzuka TV credits 青木孝司 for 撮影監督 (Photography Director), as shown in the [url=http://ja.wikipedia.org/wiki/涼風_(%E6%BC%AB%E7%94%BB)]Japanese Wikipedia page[/url], while the U.S. version of Suzuka TV credits Takashi AOKI for Camera Director, so at least the makers of the U.S. DVD translated 青木孝司 to Takashi AOKI.
The Allcinema page for 青木孝司 gives a reading of あおき(AOKI) たかし(Takashi), fueling my doubts further; Allcinema gives him credits for .hack/roots (credited to Kouji AOKI by ANN). Allcinema also has a page for 青木隆 but with two entries only (both from 2007, of which Heroic Age is credited to Kouji AOKI at ANN) and without a Hiragana reading for this name, so that may be a duplicate person entry in Allcinema as well.
Perhaps 青木隆 is simply a synonym of 青木孝司 in real life? D2_STATION has a page for Erementar Gerard that has both Kanji names in the same anime, 青木隆 being credited for 撮影 (Photography) and 青木孝司 being credited for デジタル撮影 (Digital Photography)...

Note that the mangaka/letterer/retouch artist Tomoko Saito has also used the pen name "Saito Tomoyuki" (斉藤 友之; there is a producer Tomoyuki SAITO (斎藤 朋之)), as in "Jason Thompson's House of 1000 Manga: Dame Dame Saito Nikki". Proof: the Japanese Wikipedia page for Omori Nozomi (Google Translate version):
