The second link is correct on all except using l in her last name when it should be r.

As for how companies here handled it?

Koi Kaze (Geneon) - Name flipped
Kurau Phantom Memory & Jinki: Extend (ADV) - Misspelled & flipped on Kurau, correct on Jinki
Loveless (Media Blasters) - Correct
Tide-Line Blue (Bandai) - Misspelled last name
