There are two people named Takahiro Miyake, and both are seiyuu (how should these be disambiguated?). I wasn't sure on the Blood-C credit, so I checked the ending credits myself to confirm it.

By now there's also a credit for Guilty Crown but I couldn't figure out which Takahiro it belongs to.

Just checked both episodes on funimation.com, and it's the newer (三宅貴大) Takahiro Miyake. |

The Takahiro Miyake entry has gotten a number of new roles, all of which I've identified as the newer seiyuu 三宅貴大. I've split those roles into a new entry.
