Yoshiki Yamakawa
Yoshinobu Yamakawa

I've come across both of these names in connection to the recent Kill Me Baby staff announcement and did some research. 演出@wiki claims the person named 山川吉樹 is actually read as "Yoshinobu". However, the given name kanji is commonly read "Yoshiki", and "Yoshinobu" doesn't even appear to be a valid reading. Not only that, but Madhouse's website also clearly uses the "Yoshiki" spelling. Yoshinobu makes more sense for 山川良志信, which appears to be the name he used in the mid 90's. The source for the all-hiragana form of his name (from the Fobia OAV and currently listed on Yoshinobu) isn't available anymore, though searching Japanese Wikipedia does show one instance of it being used for Debutante Detectives Corps (but not listed in ANN Encyclopedia yet). "Yoshiki" credits start in the late 90's, and apparently the Di Gi Charat and Azumanga Daioh credits under Yoshinobu were ones where he was credited as 山川吉樹 in Japanese (checking the R1 DVD release of Azumanga Daioh his name is given as "Yoshinobu" for storyboard on episode 12 and 18, but as "Yoshiki" for the ED animation).

There is a twitter user 山川吉樹 (YOSHINOBU29) which would verify the connection, if it's him (doesn't seem to say anything about anime/animating on quick glance). Whether he uses an odd reading or its just his former name is more up in the air, but the Madhouse listing has me leaning towards the latter and suggesting an alias rather than a purge of Yoshiki credits.

Since the two names are clearly the same person I aliased Yoshinobu into Yoshiki. As to whether some of the credits are not under the correct name, I'll leave that to someone else.
