So how do they get ideas for the show, from what little I've seen it's set in a small village with a little girl doing whatever. What can you do with that premise to last 45 years.

It appears that you can do an awful lot with that simple premise, if you are creative enough.

I'm sure the only way this show will ever come to the US, especially this upcoming episode, is if it airs on TV Japan. For those of you who don't know, TV Japan is a channel owned by NHK, and serves as their answer to BBC America.

Likewise, PBS has been able to make Sesame Street fun for a long time. It's possible that with thousands of episodes have been aired, certain episodes are almost/the exact same as others.
