I've participated in my first Doujin event just last month (started my own circle and everything) and it was a lot of fun even if I didn't sell as much as I'd hoped.
The paperwork and stuff is hell, however if you don't know Japanese, so a tour offering to guarrentee you a table plus translate your work for you is totally an awesome deal.
Just don't go into it thinking you'll actually sell many copies, and more as a fun experience to take back with you. Trust me that being behind the tables is a lot different from lining up in front of them.

And people in America can't publish their own dojinshi state side? Why does it have to be in Japan? That's going to great lengths, probably very pricey....

There are local anime conventions, but none of them are dedicated to people selling self-published work, as far as I know.
Besides, it's Japan, for crying out loud. I imagine there are countless artists who would like international exposure.

They still can. More importantly, they can do it in both places.

If you think people would do this because they "have to," you're missing the point of this or almost any other creative endeavor.
