the anime doesn't really answer that... and I've yet to pick up the manga. I would like to hear a definite answer to that... or ideas would be nice..

I don't know if I did the spoiler tag right or not...
I could be wrong here, but my understanding is that he used his alter of speed to speed up his metabolism and healing. So basically, he was so close to being dead that most people couldn't recover from it, but his alter alowed him to recover before he died.
Oh and just so you know, the manga has a VERY different story. There are many similarities, but mostly it is not the same. So i don't think the manga would answer this question, but i have yet to read the last voulme.
I finished this series last year, so bear with me.
I believe Straight Cougar underwent that mainland power-up process that improved his alter but at the expense of his long term health. His last use of the alter sent him over the edge and he just hung on for a while before slipping off.
