It came from the PSP version. It's part of the army building for Big Boss's Outer Heaven, in the future. AKA MSX/NES part 1-2. Which I hope Kojima remakes before he retires.

That's part of the joke. Metal Gear Solid wouldn't be Metal Gear Solid without the ridiculously stupid ways of distracting the guards. And now that the cardboard box is a part of that, I feel like the game can be taken to the next level...
Watch them put in some levels with female enemies that this doesn't work on. Try it and they get disgusted, kick you out of the box, and start shooting.

But you can't KO an enemy without moving him from there. It's like telling them "I was here and you're under attack"
