Regardless if the leak is true or not, has there ever been an anime to have three animation studios work on different seasons, barring things like reboots/remakes?

There are a bunch with 2, but then which I know well is for example if you just look at the seasons only not ova/movie its date a live. It had
1st season AIC plus studio
2nd season production IMS
3rd season jc staff
4th season geek toys
And finnaly the announced 5th season will finnaly break the curse with geek toys doing it although still not confirmed

So, not MAPPA then? That's a shame. Imo, MAPPA is the perfect studio to succeed Madhouse. I like JC Staff's anime just fine. I just don't think they can handle the action genre all that well.

Strike Witches has gone through three different studios for its main seasons:
Season 1: Gonzo
Season 2: AIC
Season 3: David Production
Its spin-offs have also been handed off to different studios as well:
Brave Witches: Silver Link
Luminous Witches: Shaft
Though unlike with One Punch Man, and many other series that up with different studios over time, most of the main staff has stayed consistent between entries instead of being swapped out with each new studio.

It's called damage control. If the leak was really misinformation, then Shonenleaks wouldn't have had their Twitter account banned.

At this point, I refuse to blindly believe any type of rumor/leak surrounding MAPPA, be it positive or negative. Feels like there's such a divisive aura surrounding them that "leakers" will just say anything and people will believe them. I remember when some guy on Twitter that others called a "credible leaker" said the Blue Giant movie would be made by MAPPA - alas, that was not the case.
Though I have no real clue who could be handling it this time, specially considering the production comittee doesn't seem like they handle the series very well.

Initial D had a bunch
First Stage - Comet & Gallop
Second Stage - Pastel
Third Stage movie - DEEN
Fourth Stage - A.C.G.T.
Fifth & Final Stages - SynergySP
Legend movies - Liden
Yes the last one is a reboot of First Stage but I count it because it was being worked on at the same time as Final Stage and the first film premiered two months after..

I don't understand why a series would switch animation studios multiple times. If it happens once, I suppose you could chock that up to scheduling conflicts, but for a series to be hot potatoed between 3+ different animation studios? I never watched the second season of One Punch Man, but I didn't hear good things. It's getting a 3rd season, so perhaps this is a silly question, but has OPM's popularity subsided?

From the knowledge I have, most of the times animation studios don't exactly choose the series they make, it usually comes down to a producer from an outside company offering the series for the studio to make it.
Shingo Natsume was working on Boogiepop and Others which came out in early 2019, so I assume by the time Season 2 was greenlit, Boogiepop was already in production and the production comittee probably did not want to wait for the staff to be free to work on the second season, so they probably scavenged until they offered it to J.C. Staff which accepted it, but didn't have a good schedule at all to make it, specially since it was made with Atsushi Fujishiro's team which was the team working on Food Wars S4, Date a Live III, and Saiki K. All of which also came out in 2019.
So I assume that for Season 3 it was the same, the producers offered J.C. Staff to make the anime, but they likely refused - or didn't even get an offer period, because of the poor reception or something.
Or I could just babbling out a bunch of crap, I don't know.

Oh this reply is gonna age like milk, it's gonna be glorious.
If it was Mappa, they would have slapped their branding all over this project since the initial announcement. They love attention like no other studio.

i mean there is chance Natsume can work on Kaiju 8, better schedule, better environment. Why now he need to suffer in OPM haha

Natsume working on a Production I.G project? Hm, I frankly don't see that happening at all. The reason why Natsume even got a chance to direct so many series at Madhouse is because of the animation producer Yuichiro Fukushi being acquainted with him, I don't think he's the type of guy that any studio/producer could just get to work with.
