Unable To Add Some My-Otome English Staff Credits

When I try to add Michael Hey and Sean McConkey for "Video Post-Production" for My-Otome, I get these 2 errors.

Any idea what the problem is? I take it this info was entered in before, but then removed via an error report?

Here's the DVD credits from Volume 1, with the last picture being the most relevant for these specific people.

It looks like these were marked as false because the original submission did not have a valid source. They should now be set to where they can be entered again with the correct source.
