I checked my copies out, Cat Returns had a coupon but Nausicaa and Porco Rosso didn't. That is kinda wierd, but luckily you only need one coupon, or do you need one for each DVD?
I have disneys earlier reseases so it's not so something I would take advantage of.

Good thing all three of mine came with one (only need one). I got mine at Best Buy if no one cares. Anyway, I still need to order Kiki...

You only need one form, the receipt (or a photocopy of it) and proof of purchase tabs from the back of each of the three boxes.
The store (Anime-V) I got my copies from had several boxes without the special offer label and insert. They were careful to see to it that I got one box with the label, it was the only one with the form.

I really wish companies would think up another way rather than cutting out the proof of purchase, as it makes the collection ugly with an inch long square missing from the back of the case. an insert or something would be cool.

Agreed. I am also really upset that they ask you to cut up the box. It's almost not worth the free DVD to me, since I basically collect DVD's.
All 3 of mine had the form (bought them at Best Buy, too) and I might call for a replacement form for the one I cut up, since I'm even anal about the internal contents of my DVD's.
