This question has been on my mind for a while; to start it off, I'll explain why I thought of it. I've watched quite a lot of anime, most of them only left me with a trace of memory, nothing detailed, yet some triggered major emotion crisis.
I did some analyzing, and thought of a list of elements I'd prefer in an anime.
First of all, it would be mainly shonen and fantasy, in addition to that, there would also be slight historical and romance. In the begining, the main character would've already obtained an amount of skill through hard work, ( I really hate anime characters with extraordinary powers that came from no where). Further more, there would be lots of characters and a twisty plot that involves betrayal. (Although it could become very messy later on.)
Animes like Naruto and bleach, I would say they have good plots, but they get TOO shonen ( Long battle scene with the exact same techniques) and repetitive. I think a little romance would be nice. Also I think characters ought to
have more emotions than plain fighting spirit, applying philosophical means to the personaities would be rather refreshing.
My personal favorite of this kind is probably Rurouni Kenshin, though it lacks the element of fantasy.
I don't know, upon finishing explaining things, I find my head spinning and not to mention feeling kind of stupid for asking such question. (Because it feels like another way of asking " What's your favorite anime?".) To clear things one last time, I'm not only asking what's your favorite anime, I also want to know WHAT you prefer if you were able to create an anime this instant.
So what's your kind of adventure?

Ever since I've gotten into anime, I've surprised myself with the diversity of my tastes. I've found something I like in just about every genre, and ranging from decades-old to recent. That said, there are specific things that I enjoy in anime, and the perfect show would have many of them.
Story is important; one thing that keeps drawing me back to anime is the fact that it does not lack interesting stories. The story of a perfect anime does not have to follow a set pattern in my mind, but the story should surprise me; I want to be able to look back at the series/movie afterwards and find that it appears to be a completely different piece once I know what's really going on.
Characters are important; not just character personalities that I enjoy watching, but rather people who develop organically with the progressions in the plot. Once again, my mantra is surprise. The characters should not be entirely transparent from the beginning, they should leave me at least partially in the dark about their nature.
I'm in a minority concerning this aspect, but music is important. I enjoy the subtle boost a perfect soundscape gives an anime's atmosphere, that intangible combination of necessary elements.
Of course, since this is anime, visual quality is important. If the creators have a low budget, then they can circumvent their problems through excellent cutting and storyboarding. The visual style does not have to confirm to a certain set of rules, but it must be consistant with, and a complement to, the show's overall tone.
I'm sorry that it's a vague answer, but my ideas of a perfect anime are not a strict set of rules, but a stringent set of guidelines.

For me the answer to the topic question is easy, my idea of a perfect anime is one that is entertaining but has a very well done plot with good characters. The actual animation is important but overall it's the story and characters that move the show along and keep you watching. A show can be as crisp as can be but if the plots sucks and the characters are dooshbags you won't watch it for long. Obviously the plot has to be entertaining, and by that I don't simply mean filled with action. A good thriller or drama piece can be entertaining. By entertaining I mean it keeps you interested. A good story and characters will keep ya seated but the show also has to be entertaining for continued viewing. So basically just make the story top notch, and worth watching and the characters well drawn out and it's perfect for me. I tried to keep this short and sweet.

I generally like series where the characters are innovative and human.(meaning no perfect heroes and whatnot) Also, it's just a personal taste, but I like mecha(as long as it's not focused specifically on the mecha, and as long as there's no invincible mecha that kills everything) and an original plot. Add some philosophical elements and nicely paced drama and that would get me hooked.

My guilty pleasure is when the hero/one of the heroes/group of heroes is severely underestimated by someone around them or the public in general, and then reveals their true power/talent much to the shock of the person/people around them. If that occurs in a show, I'm sold, unless it was cheesy or otherwise poorly handled.
If I were to envision a "perfect anime", this plot element would be in the show somewhere.

Fullmetal Alchemist comes pretty damn close to my ideal anime, except for the fact that the element of romance isn't present. However, it has everything else:
An imaginative and fantasy-filled setting, characters who achieved his status through hard work, an amazing plot, emotional conflicts and situations, outstanding character development (you actually see Edward mature throughout the series), amazing plot twists, great animation and battles, awesome character design, comedic moments, and creativity (just the concept of alchemy is so well done.)
So my only problem is no romance, but to integrate something like that into FMA would be very difficult.

Mine would be a Fastasy- Record of Lodus War- style of show. I love sword fights and Mid-evil settings. They are so rare to find a good anime with that style. Have a hero something like the one from Berserk with a I don't realy care attitude swinging a big sword around. While having a suporting char that has some inteligence but is more along the lines of a sneeky person, not realy a rush in ad fight. I wouldn't have much comedy in it, if any. I prefer a serious type of show. Would defidently have to be travleing, the same area can get kinda boring. As for the lenght of the show, I would put it at 24-26 episodes long. Just long enought to cover a good deal of the story and get somewhat in depth with it, without drawing it out and streaching for content and having fillers. I'd only make it long (40-50 ep long) IF I thought i could do it without runing the show.
As for VAs I'd would like to have the VA that dose Uhraha from Bleach ( sorry didn't want to look up his name), but he seems to be more comical. I also like the person that dose Bakuya from Bleach, he has seemd to have done more serious roles. As for the rest, I'm not too sure, just have to hear people try out i guess.
The story would proress and end something like Cowboy Bebop, love it when the "hero" dies. Would have some back story to the main players in the story, but nothing to lenghty, or prehaps put out an OVA for the back story alone totaling around 3-6 episodes depending on how indepth i wanted to go.
Music would be a mixture of hardcore rock music and classical depending on what was happening in the scean. For opening and closeing credits I'd go with a hardcore sound. I wouldnt want the music to be lyrical, except for the opening and ending, because it can draw away from the show.
The annimation style...I would to have Mad House as the animators for it, or someone with a similar style. I love the detail they put into it.
And that folks is prety much my perfect anime.

my perfect kind of anime i dun really care if there is any action at all...
honey clover + nodame + ouran host club... those are about as close as it gets of my definition of perfect
good comedy... they don't FORCE the laughter out of u... rather soothing and comfortable to watch...
if i can't sit and watch the anime for 10 hours straight... then it has elements missing for a perfect anime... doesn't bore me and keeps me wanting more >_>

First a gorgeous and unique world for the setting, since I don't have any example in other anime, I'll use a video game one; à la Bioshock, Crimson Skies etc, etc. Story and atmosphere should be very important.
Second I want heroes that aren't nobodies. And the main protagonist should be a not-so-nice-person, albeit a very intelligent and cultivated one, a**holes make the best character and leave a lot of room for interesting character development. So, maybe something like a general from an evil empire, his superior want him dead because he's getting too high-ranked, they fail, he wants a revenge, he joins freedom fighters.
Then if there's fighting it mustn't be too brainless, they have to make up strategy if they even want a chance, every combat should be physically and mentally challenging. Also some character could be injured permanently for the rest of the series, and those injuries should not necessarily be inflicted by the antagonist it might have been done by grunt that has five seconds of screen time. It would feel more realistic like that.
A couple of things I want:
No magic or supernatural.
Betrayal by someone unexpected, that person should succeed in killing at least one important or semi-important character.
Last fight should be a desperate one with only a very slim chance of survival.
The noble class gets a senseless beating. I don't know why I get a kick of rich people suffering. I'm a bad person.
.... There's probably other thing, but that's all I can think of right now.

Well first off to be MY perfect anime i has to have a strong, independant, beautiful, funny, smart girl. NOT a perfect girl because she need faults like being lazy or too messy or can't cook. She can't be shy and she asserts herself or her feelings straight forward. Also never scared of anyone even big guys because she can take them down. A lead character with CHARISMA! Character charisma draws people into some anime a'la Haruhi or Nana Ozaki.
The supporting characters have to be just as interesting with colorful pasts and personalities different from the lead in major ways. A'la Hachi and Nana are almost polar opposites but not quite. The anatagoist can't be you're sterotypical antagonist. An example of a great antagonist is Sae of Peach Girl. I never seen any antagonist succeed or fail to THAT extent.
I perfer anime to be set in the "real" world. Basically present day time to us.
The plot needs to be funny as hell. It also needs to have a consistent flow like Esacaflowne. It doesn't matter what setting in the "real" world but as long as the characters actions, thoughts or comments bring comedy. a'la in H&C with the kokoropur fiasco or Genshiken with Saki's beatdown of the members It can have a bit of drama to add spice and maybe a fight now and then but it needs romance! Not a lot but some. In short:
1/4cup - romance
3cups- comedy
2 tablespoons of action
2/4cups- drama
Seasons of weirdness and situations to taste.
Oh yeah it has to be way different than most anime. It can't use a bunch of cliches. One is fine but its so obvious if you use all of them (ex: clumsy girl, studious girl, boy with big sword, magical girl not that there anything wrong with that!)
The animation must be stellar. If anyone had to animate my perfect anime I'd pick Kyoto animation. What they did with Haruhi and Air was amazing! (although I really hate air but its so pretty!) I guess because I study animation seeing high quality is like me exploding! The character designs must be cute but not super moe cute. Just cute.
The soundtrack must vary from super pop hit to original anime sounding song from seiyuu. As in you can have a super anime OP but say an ending from Utada Hikaru's 3rd album.
And unless I think of something else today this is it.

I can't really say what my 'perfect' anime would be because I enjoy so many different genres, and if you were to stick all of my favorite anime in a blender together the result would be rather frightening. But if it's going to be done...
I guess I'd want the smooth plotting and action of GitS: SAC, the characterization of NANA, the dreamy feeling of Haibane Renmei, the slapstick comedy of Haruhi Suzumiya or Galaxy Angel, and the lighthearted, slighly satiric warmth of Shaman King, all set to a soundtrack comparable to that of Arjuna.
I do like fantasy, though. And computerized gadgets, gotta have those. Oh, and guys with long hair and big-ass swords. And two side characters, a guy and a girl, who have sort of an understanted romantic relationship a la Roy and Riza from Fullmetal Alchemist. No really obvious romance, though, although I would like lots of pretty girls who can hold their own in a fight and kick some ass. A corrupt government would be cool too. Also, the main character should be voice acted by Paku Romi in the sub and Crispin Freeman in the dub (strangely, I can see that working.)
....So, how physically impossible is that? :p

I have to agree with frenty, when the main character is surrounded or underestimated, and then reveals the true powers within, that's where you get to see some kickass action. Although when you have a badass main character, it's fun at times also to see him underestimate an enemy as well. That's what is so fun about Hellsing Ultimate. That to me is a perfect anime. Though we have to give credit to comedy as well. I'm not just one who likes blood and guts and all that nice stuff.

I have to say, creating female characters is way harder than those with male characters, (especially if it's the main character). I think girls are compicated creatures, (not saying guys aren't..>>), their true personalities can be well hidden yet they tend to show more emotions on the outside. However, it's quite difficult to bring out the inner beauty without being too cheesy. So a good anime should most definitely have baffling female characters despite its subject (which I find many animes fail on that account)
There is one more thing I'd like to add which many have already touched upon.
Although each character should be unique as an individual, they should also correspond to one another in whichever way, whether it's the protagonist or the antagonist. The right combination of character will do the trick, they work together as a unit to bring out each concealed element in consistency, well in the mean time, also leaves part of the mystery untouched.
