Tsubasa Haduki
Tsubasa Hazuki Yen Press as using "Haduki", although there are versions of the SAO: Fairy Dance vol. 1 cover that use Hazuki: 
Both the "du" and "dzu" spellings of づ are deprecated in Hepburn romaji so if we were sticking regimentally to that scheme Hazuki would be correct, however there is clearly a reason why Yen Press decided to change the spelling on the cover so we can assume that is the preference of the author/publisher. |

There are many cases where romanizations by the person him/herself and different domestic / foreign companies vary significantly. With merging being reversible I'd say it's safe to stick with one's own romanization if available, followed by publisher / licensee's choice.
