With all this delays no wonder this one got an OVA instead of a TV series, unless they pushed this project into an already busy scheduale that they had so that they can afford then delays with an OVA if it was needed which end seemed so.

Shaft's seriously going to burn down this year with how much stuff they plan to release: two more Zaregoto volumes in the next 4 months, Owarimonogatari vol. 2, the second season of Sangatsu no Lion and as soon as the year ends and the next one begins, Fate/Extra. I'm sure we'll get to enjoy a buttload of production problems for many of those series in true based Shaft fashion.

I die a little inside every time I hear the word "circumstance" in place of issue. I also die a little every time I hear about production issued which translate to, "we can't get their house and production schedules in order", something which seems to be occurring with more and more frequency in the industry over the past few years. If you can't manage an OAV production schedule, it leaves very little doubt in my mind, what the house looks like for managing a TV production schedule.
This isn't rocket science, this is the core of being a studio. If you can't manage the basic, you might as well be in the street selling hot dogs or running a pet grooming salon. I'm not so much outraged as I am annoyed by the fact that production studios are trying to run too lean, and not having enough staff on hand to balance the scheduling and workflow requirements of their business. This isn't just a Shaft issue, all the studios are doing this.

Quality control is definitely something that could be improved in many industries. A think a defining example of this was all those delays for "The Last Guardian" (several years) until it got released last winter.
