There appears to be another Sanae Yamamoto (#19849), since about half of the works credited to him on his page were produced after his death in 1981. Notably (but not limited to), your name., Up on Poppy Hill, Garden of Words, and Otona Joshi no Anime Time.

Kara once stated in a tweet that her 5th favorite character of the characters she played was “Nanami”. The “Nanami” she is referring to is Nanami Shinonome from Dance in the Vampire Bund, as Kara did not play any characters named “Nanami” using her real name.

While “Christina Moore” may be how she is credited “on the dvd” (as sourced), the ending credits for episode 29 list “Christina J. Moore” for the role of Hera in GetBackers.

In the Pikachu’s Jukebox section of the Season 2 Pokemon credits, an “N. Grossfeld” is listed. This entry represents Norman J. Grossfeld’s songwriting credit for “What Kind of Pokemon Are You?” (as seen on the 2.B.A. Master album), one of the songs that play during that segment. Russ Velazquez also refers to him as “Norman Grossfeld” on his upload of the Sonic X theme (whereas “Norman J. Grossfeld” is credited for Theme Song with Velazquez in that show's ending credits)

XII. Russell Velázquez (person#30007) (correct spelling) and Russell Velasquez (person#66117) (incorrect spelling)
“Velázquez” (with 2 ‘z’s) is the correct spelling of his last name, as per his YouTube.

As seen in examples in this vid such as 0:09, 0:38, 1:09 and 7:23, it has always been Ryan Kelly in One Piece's credits.

XIV. Paul J. Baccash (person#12020) (more complete name), Paul Baccash (person#12506) (variant) and Paul J. Balcash (person#50450) (erroneous spelling)
As seen in same One Piece credits vid, Paul Baccash (0:09) later changes to the more usual Paul J. Baccash form (7:22). “Balcash” is completely wrong, though, as seen from these 2010 airing credits for Magical DoReMi (0:14).

XV. Rita Rivera (person#38974) (correct spelling) and Rila Rivera (person#30468) (erroneous spelling)
One last foray into the 4Kids One Piece credits vid for now (0:06, 0:35, 1:07, 7:20).

XVI. Jerry Lobozzo (person#12005) (full name) and Lobozzo (person#7904) (last name variant)
Between Seasons 3 (Johto Journeys) and 4 (Johto League Champions) of Pokemon, no one in the voice cast was dropped or added. Instead, a few names were changed/updated. “Addie” (Blaustein) became “Maddie”, Ikue Otani’s last name spelling changed to “Ohtani”, and Lobozzo’s first name was added to his line.

XVII. David Brimmer (person#1073) (real name), Michael Alstin Baley (person#19363) (variation of alias), Michael Alston (person#55671) (variation of alias) and Michael Baley (person#66143) (variation of alias)
Brimmer (as MAB) appears on the far end of Samurai Deeper Kyo as Nobunaga Oda using a deeper voice [ala Odion from Yu-Gi-Oh!] than what he gave Genma Kitoh (“Alstin”). Speaking of Odion, “Michael Alston” is how MAB is credited in Yu-Gi-Oh DM's final season (for Odion and/or Shada). Given the flexibility of this alias to be broken up as it has been and Brimmer’s unmistakable vocal stylings and presence, GaoGaiGar‘s Michael Baley can be concluded to be him as well.

XVIII. Michal Friedman (person#51785) (real name) and Michal Meadows (person#21760) (alias)
As mentioned in the site’s article reporting her passing, Michal The Girl had included note on her website of her having played Saisei in Samurai Deeper Kyo and various roles in The Gokusen – the roles done by Meadows.

XIX. Michael Liscio, Jr. (person#124913) (full/main name), Liscio, Jr. Michael (person#124912) (incorrect name order) and Michael Liscio (person#154215) (without Jr. variant)
Commas turning suffixes intro honorifics, and “Michael Liscio” was particularly used for Sm4sh, where Liscio reprised the Pokemon Inkay.

XX. Eli James (person#100775) (full/main name) and Eli Jay (person#105480) (variant)
James’ resume includes Yu-Gi-Oh! Zexal protagonist Yuma Tsukumo, thus Jay is short for James.

XXI. Joey D’Auria (person#11942) (real name) and J.W. Terry (person#142486) (alias)
This was revealed by the official Yo-Kai Watch Twitter here, explaining that they were one in the same. His real name has since been used for Yo-Kai Watch: The Movie.

Tony Rodriguez (person#3756) is a duplicate and a mispelling of Toni Rodriguez (person#71081). She's credited as the latter name (example: http://es.doblaje.wikia.com/wiki/La_Cenicienta_II%3A_Un_sueño_hecho_realidad#Cr.C3.A9ditos LAS dub credits of Cinderella 2) and her Facebook page has that name.

The difference of a dot is not a factor [nor the voice, as heard later]; the earliest of the not-dot works (Kurai in Angel Sanctuary) actually had a dot, as seen in the ending credits (which this review [16:49] just barely leaves in). The rest of the non-dots (and one of the with-dots) are all tied to the role of Ikki Tousen’s Gentoku Ryuubi. Despite the name use, Taylor has liked some tweets that feature her, such as this clip.

The role of Dengaku Man in BoBoBo-Bo Bo-BoBo is credited to Donn A. Nordean, as seen in the credits of later episodes (0:19). I. Ignacio Lorefice (person#148581) (main name) and Ignacio Loreficce (person#155161) (erroneous spelling) are the same person. He was credited as his main name in the first season of YKW and as his alias in the second season. Examples: YKW S01E02 (from season one, as Lorefice) YKW S02E02 (from season two, as Loreficce)
II. Juan Echave (person#155162) (shortened variant) is an alias for Juan Manuel Echave (person#146817) (full name). He's mainly credited under his alias but some works credits him as his full name. Examples: YKW S01E08 (from season one, as "Juan Echave") Birdman LAS dub credits Desmond Argentine Spanish credits (non-anime roles, as Juan Manuel Echave)
III. I've recently discovered that Toni Rodriguez (person#71081) was credited as Tony Rodriguez (person#3756) in the Evangelion dub credits. The pages Pietro Mario and Mario Pietro should be merged, as they're about the same person. The correct order is Pietro Mario. Additionally, it should be "Mário" with an accent.
