I was actually thinking it was Souji Okita, but I just didn't know from which anime it was from since he has been in multiple anime along with his tuberculosis.

Same, actually. I felt pretty good about him being based on the historical figure in question, but I was guessing at the anime. The only thing that kind of narrowed it down for me was that the image appeared to be from an older show. Upon further research, I may still have been slightly off, because I'm thinking it's not from the TV series, but the Trust and Betrayal OVA.
Hint 1: In this scene, she is imploring the title character not to think badly of her cousin, who had made some unfortunate first impressions.

The main thing that has been keeping me from guessing is the lack of information I can find about the anime as opposed to the game, but I'll go ahead and guess anyway since no one else is.
Is it Altera from Manga de Wakaru! Fate/Grand Order?
