These two entries are actually for the same seiyuu, who changed her first name sometime around 2007. For sources, see the Ao Takahashi MauMau database page, which lists roles for both names, or her Japanese Wikipedia entry, which states that she previously went by Hiroko Takahashi (旧名および本名は高橋 裕子（たかはし ひろこ）。).

Done; #14004 is not exactly related (either that or another staff split the name first) but has its own problem.

I think something went a little wrong with the cleanup of her roles. On the page for Ao Takahashi, she is now listed twice for three roles, once under the name "Ao Takahashi" and once under the name "Hiroko Takahashi".

Yes, some additional cleanup is needed. She's still credit twice for the role of Karin on the Kita e. anime's page, for example - once as Hiroko Takahashi and once as Ao Takahashi.
