This is the disc 5 replacement for the ultimate edition of GKIDS' Blu-ray release of Neon Genesis Evangelion. A unique SKU was printed on the packing slip included in the mailing envelope.

Notes:
Contrary to what the GKIDS site says, the front of the replacement disc is identical to the original disc. The discs can still be uniquely identified by the code printed on the inner ring of the data side of the disc. The original discs have the text 58063755/ANI0633-ANI0500, whereas the replacements have the text (1189821) 2000176066.
