I've long suspected that what has slowed down this much-rumored project is the distinct possibility that Jun Maeda has simply run out of ways to kill teenage characters.

After all, there are only so many possible permutations of freak accidents, vehicle crashes, mass transit disasters, centuries-old curses, and unnamed mystery ailments to go around… and that's even after Maeda introduced "violent home invasion" to his character-killing repertoire.

Lol it's not so much about different ways to kill off the characters as it is about making it seem unfair to the point where they feel like they have to fight against reincarnation, and indeed life itself.

I disagree. Maeda doesn't need to come up with any new ways of killing them off, because, as you noted, he just reuses the same ones every game anyway. Laughing Since Rewrite had some actually original tragedies... I fully expect them to be reused in the Angel Beats! adaptation.

Maybe he can do super mundane deaths, like...choking on a hamburger or falling off a ladder.

Oh wait, he already did "death by falling" with Naoi's backstory.
