Berserk has quite a unsuiting opening and ending theme.
I first thought I had The Pillows or FLCL (aka: Fooly Cooly) running in the background but to be honest, pop does not fit a dark mistery, middle earth, supernatural, fantasy anime; the music is way off

Haven't seen Blue Sub in a loooong time, but I think I really liked the soundtrack when I heard it. Then again, I'll actually choose to listen to jazz on my own on rare occasions, so I'm probably just a budding fan or something.
I also prefer Maximum the Hormone's Death Note music to Nightmare's. It's so... destructive and chaotic, it fits the overall atmosphere of the series and just sounds awesome to boot.
I really can't think of any anime where the music was so bad that it affected the experience. I... guess I got a little bothered by .hack//SIGN's soundtrack when I wasn't in the mood for it, especially with the repetitiveness, but I guess I just haven't found/don't remember any anime where it was really bad.
I'm sure there were some specific scenes with tracks that effected the scene, but no really overall bad soundtracks.
Now some video games on the other hand...

I second the bad music on Blue Submarine No. 6, it was generic jazz which sounded like coming from an automated synthesizer. Good show, though.
The music in Kanon is really annoying, too, it sometimes doesn't fit at all and even if it fits, it isn't good.
The music in Claymore and Full Metal Alchemist does not live up to the standard, either. Although I wouldn't call the music "bad" exactly, I think they would've supported the show better if they could stand on their own, much like the Final Fantasy OST do, or most music of Yoko Kanno and Yuki Kajiura.

FMA's music was written by Michiru Oshima. I'm not terribly familiar with her overall work, but I really didn't think the music in FMA was very impressive either. But...there was a single piece which not only impressed me after I listened to it outside of the anime, but impressed me while I watched the scene. Near the very end (Episode 50 maybe?), Ed enters the underground chamber, kind of ballroom-esque, and this odd waltz is playing in the background. It sounds standard enough at first, but upon closer listening, the melody reveals its unsettling nature. In the anime, this is accompanied with the movements of Rose, dancing alone as if possessed. It was the single most affecting scene in the series for me.[/spoiler]

Anime music usually doesn't bother me that much. I generally avoid the shows with bad musical talent for their content anyway (they seem to sort of go together).
One exception that annoyed the hell out of me though was School Days Episode 12. They completely set the viewers up for the crazy psycho killer scene and then out of nowhere blares this annoying Kanaaaaaashiiiimi nooooo mukooou he to!!!!!!! etc. sappy emo song to the extreme.
I mean, I understand that crazy hardcore otaku sitting in their basements who've played through the School Days game and gotten every single ending would jump for joy and say "hey, it's the song that plays during the bad end, omg!" but that and subsequent scenes would've had 10x more impact with the inclusion of psycho horror music, just like the music they used in a previous scene of that episode when Makoto and Kotonoha start making out in front of Sekai

sometimes, the music affects me while watching anime...
i liked the whole anime of death note and all.. but omg! there were a few themes that were like wtheck?! O.o" i dunno, i think its called hard rock or something.. but wow, it was so "rock-ish" that i was like skip, skip!! not my type of music here...
and then, recently, vampire knight came out right? i love the beginning theme, but the ending theme was so ehh? it was too "emo" for me.. maybe something more happy would be okay..
there were also a few others, but can't think of them at the moment, so yeh..

I usually like songs in OP/END probably cause most of the time I just skip them.... Well...sometimes even if I like the song but they keep playing it over and over again for the whole anime without changing an OP/END theme it just drives me mad... I don't really mind if there is 'not my taste' song in anime as lonng as they don't play it too often or will change it soon.

It's a variation on Dante's theme (from the second OST), but I'm not exactly sure what the track is called. I do love the melody, though, and the many versions of it on the otherwise underwhelming FMA soundtrack. It and Bratja are really the only reasons to get those OSTs.

A lot of the time my first impression of an anime is based soley on it's opening song and how it fits with the animation (I know, it's somewhat stupid but I can't help it). I can't recall any specific anime, but I'm sure that I've given up on watching a series because of a terrible soundtrack.
